Phaco-emulsification in A Patient taking Tamsulosin ( Voice -Over )-DR SUDHIR SRIVASTAVA

The use of tamsulosin to treat prostate enlargement and urinary symptoms was linked with an elevated risk of complications after cataract surgery. The uroselective alpha-blocker tamsulosin is the most commonly used drug among all. Studies showed that the majority of the patients who develop intraoperative floppy iris syndrome (IFIS) were on tamsulosin.

Anterior Chamber Bleeding during Phaco-emulsification -(Voice-Over)DR SUDHIR SRIVASTAVA

Anterior Chamber Bleeding during cataract surgery is not common, but patients on blood thinners, Iris -neovascularization and iatrogenic during surgery might be the reasons. Use intracameral Epinephrine a known vasoconstrictor and raised anterior chamber pressure for some time would help to stop the bleeding.

Divide And Conquer Phaco-emulsification – ( voice-Over) DR SUDHIR SRIVASTAVA

Divide and Conquer is a type of Phacoemulsification technique for phaco fragmentation . Good for beginners, and the best way to know the Geometry of Nucleus. However, it requires high power than other chopping techniques. Crisp division is key to success.

True Vision NGenuity 3D System For Eye Surgery -Review -(Voice-over)- DR SUDHIR SRIVASTAVA

The NGENUITY 3D Visualization System was developed in collaboration with TrueVision® 3D Surgical, a California-based leader in digital 3D visualization and guidance for microsurgery. It consists of a 3D stereoscopic, high-definition digital video camera and workstation to provide magnified stereoscopic images of objects during micro-surgery.

Reason For Persisting Corneal Edema After Cataract Surgery (Voice-Over)-DR SUDHIR SRIVASTAVA
A small Nucleus Fragment was found in the Anterior Chamber. Therefore a careful Slit-lamp Examination becomes important to detect such situations, although corneal edema makes it difficult.

Can A Phaco-Surgeon Perform A Good Glaucoma Surgery ? (Voice Over) -DR SUDHIR SRIVASTAVA
A combined Cataract and Glaucoma Surgery is always a preferred approach.. it’s possible to have cataract surgery if you have glaucoma. In fact, in some cases, cataract surgery can lower high eye pressure, reduce the number of medications you need to manage your glaucoma, or possibly eliminate your need for glaucoma medication altogether.
Vertical chopping method for efficient phacoemulsification(Voice Over )-DR SUDHIR SRIVASTAVA

Vertical chopping techniques ; After the center of the nucleus is impaled with the phaco tip using high vacuum and burst mode, a chopper with a sharp tip is buried within the nucleus, just adjacent to the phaco tip. The phaco tip lifts while the chopper depresses and the instruments are separated to effect the chop, which occurs along natural fault lines in the nucleus.

Phaco-emulsification in a patient with Iris Coloboma (voice over )-Dr Sudhir Srivastava

A coloboma (from the Greek koloboma, meaning defect) is a hole in one of the structures of the eye, such as the iris, retina, choroid, or optic disc. The hole is present from birth (except for one case, where it developed within the first few months of the child’s life) and can be caused when a gap called the choroid fissure, which is present during early stages of prenatal development, fails to close up completely before a child is born.

Capsular-rhexis went out of control ,what to do now?

Creation of the anterior capsulorhexis is a key stage in cataract surgery, and producing a precise rhexis is of paramount importance. The most commonly used technique to achieve this during phacoemulsification is continuous curvilinear capsulorhexis (CCC).

mistakes in PCR (posterior capsular rent) management during phacoemulsification

While doing a routine cataract surgery by phacoemulsification, I noticed a large posterior capsular tear while doing epinucleus removal. Vitreous was undisturbed, I tried to remove the cortex using blunt tip sinskey’s hook and I/A probe (should be avoided). PCR was large so single piece PCIOL could not be placed. Multipiece IOL was placed in sulcus. Pilocarpine was instilled, pupil was round at the end of surgery.

Cataract Surgery (Phacoemulsification) in Closed (Miotic) Pupil

Small pupil may be encountered with posterior adhesions (synechiae) between the iris and the crystalline lens. As isolated pathology, it is usually a result of long-term use of miotics, pseudoexfoliative syndrome, or neurological disorders. Posterior synechiae is a result of uveitis, acute angle closure glaucoma, ocular trauma, or previous surgery. Removal of a cataract through such a pupil is challenging.

Modified L-Shaped Scleral Incision to reduce incision Induced Astigmatism

High astigmatism is an important cause of poor uncorrected visual acuity after cataract surgery. Incision along with limbus gives more impact on corneal toricity ..

Implantation of Tecnis Eyhance IOL with excellent distance and Intermediate Vision

Today most lenses in the monofocal category only correct vision to help patients with cataracts see things at a distance, and thus do not improve the intermediate vision that is required for many important daily tasks. To better meet the needs of today’s patient, Johnson & Johnson Vision introduces TECNIS Eyhance IOL, the first monofocal IOL in Europe to provide high-quality intermediate and distance vision.10 TECNIS Eyhance IOL allows patients to achieve significantly improved intermediate vision, compared with a standard aspheric monofocal IOL, along with 20/20 distance vision.11

Modified Pocket Clear Corneal Incision gives perfect closer

Since the introduction of sutureless clear corneal cataract incisions, the procedure has gained increasing popularity worldwide because it offers several advantages over the traditional sutured scleral tunnels and limbal incisions. Some of these benefits include lack of conjunctival trauma, less discomfort and bleeding, absence of suture-induced astigmatism, and faster visual rehabilitation. However, an increasing incidence of postoperative endophthalmitis after clear corneal cataract surgery has been reported. Different authors have shown a significant increase up to 15-fold in the incidence of endophthalmitis following clear corneal incision compared to scleral tunnels. This modification gives perfect rock hard anterior chamber stability after cataract surgery

Phaco-emulsification in vitrectomised eye

Cataract formation is one of the most common complications of pars plana vitrectomy (PPV), with rates varying from 12.5% to 80.0% reported in the literature. … However, because vitrectomized eyes are anatomically different from unoperated eyes, they are at increased risk for intra- and postoperative complications.

Comparison Of Dispersive And Cohesive OVDs ease of removal (Voice-over)DR SUDHIR SRIVASTAVA

This video shows a comparison between Dispersive and Cohesive OVD. Dispersive OVD is harder to remove from the eye since it coats the endothelium and ocular structures. Cohesive is thick and viscous, it stays in the anterior chamber and maintains anterior chamber during intraocular manoeuvres, however easy to remove through IA .
